Players must call and verbally commit prior to sending in registration forms so we can confirm availability. Contact: John Novak at 888-313-7469 x 102 to commit. After you've verbally committed, please print and complete the registration and medical release forms below. If paying by check or money order, please mail to the address provided on the registration form. If paying online by Paypal, you may fax your registration forms to 888-313-7469. We'll send an email to confirm receipt of your payment along with additional instructions for the camp.
In order for registration to be complete, we must receive your registration form. Payment alone will not secure your spot. If you do not receive email confirmation within 24 hours after registration is faxed, or 7 days after it's mailed, please inquire. Since there are a limited amount of positions available, it is highly recommended that you register early.
